Health systems and providers across the country have been deploying telemedicine to safely screen and treat patients for COVID-19 and to try to contain the spread of the virus while offering all patients access to the health care they need. Telemedicine got an additional boost under the $8.3 billion emergency funding measure from Congress, which loosened restrictions on its use to treat people covered under the federal Medicare program and increased payment to providers. More medical stop-loss insurers are imposing lasers on self-funded plan members to limit their exposure to catastrophic claims. With a laser, the carrier either totally excludes certain individuals from stop-loss coverage or raises the deductibles for those individuals, with the plan sponsor picking up the tab for claims below the higher deductible. As an alternative, some stop-loss carriers are offering “no-laser” policies at renewal, contracts which often come with a hefty price tag. However, self-funded employers have a third option to either picking up the tab for lasered plan members’ medical expenses or paying for an expensive no-laser contract: They can forming a group captive to fund the difference between the plan’s medical stop-loss deductible and the laser level, thereby spreading this cost on a quota-share basis among other captive members. This session will explore how to structure a group captive insurance program to insure lasers; how a team-based approach to managing claims can help keep costs in check; and how using a captive to fund lasers also enables members to collectively purchase stop-loss coverage with a higher deductible, which costs significantly less than such coverage with lower deductibles.

This presentation explains how voluntary benefits can help reduce corporate employee-related insurance expenses by influencing migration from a PPO to HDHP, lowering core medical and stop-loss claims through correct plan designs. Voluntary benefits are also useful in reducing workers compensation claims that originate outside of the work environment. Approximately one in eight infants start life in the neonatal intensive care unit (NICU). Additionally, these vulnerable infants are often in the top 1% of an employer’s total medical spend. When faced with an infant in the NICU, both the employer and the employee’s family experience unexpected challenges. The “Diabesity” epidemic (obesity and type 2 diabetes) is fast becoming one of the largest pandemics in human history. Today, more than 100 million U.S. adults are living with diabetes or prediabetes, according to the Centers for Disease Control. Diabetes also has become one of the major cost-drivers for self-insured employers: Healthcare costs for people with diabetes are 2.3 times higher than for those who do not have the disease. But actively managing this chronic condition while improving patients’ health literacy is showing promise as a successful antidote, based on the experience of at least one employer. The COVID-19 pandemic presents a unique circumstance where workplaces that were not typically considered hazardous have suddenly become very dangerous for workers. In response, at least 14 states have expanded workers compensation benefits to cover COVID-19 as a work-related illness for certain types of front-line employees including health care workers, mass transit operators and grocery store clerks. This presumption places the burden on employers to prove that the infection was not work-related, making it easier for those workers to file successful claims. As such, employers have raised concerns that these presumption laws will increase work comp program costs at a time when businesses are already facing significant financial hardship due to the pandemic. This session will explore the impact expanded work comp benefits for COVID-19 infections is having on self-insured workers compensation programs. What is the frequency and severity of COVID-19 claims? How much are they costing self-funded employers? How do the treatment protocols for COVID-19 compare with other types of occupational illnesses? What protocols should employers deploy within their return-to-work strategies? PBMs come in all shapes and sizes, and each has its reasons for putting particular drugs on the formulary it promotes, but new data shows that there is costly waste in many formularies now being used by self-insured health plans. In fact, evidence now suggests that there are more than 800 “low-value” drugs commonly paid for on a routine basis and by eliminating just a fraction of these would likely result in significant cost savings.
